When improvements are realized, it is important to assure that root causes of the original problems are addressed and controlled. Prevention is a key aspect of what we do. It is very common to realize a "Hawthorne Effect" and gain a false sense of security that something is fixed, only to have it break again a short time later. Understanding this and mitigating root causes is one of the more difficult aspects for organizations to "see" on their own.
